Thank you for considering MacKillop College to further your daughter’s education. Below is the MacKillop College Enrolment Form. Please print this document by choosing the 'Pop out' option, complete and return to the College along with the following documents:

• Copy of Birth Certificate
• Copy of your daughter’s last NAPLAN results
• Copy of your daughter’s last School Report 

And if applicable:

• Original current Family Court Orders (These documents will be copied and returned to you.)
• Original Visa or Citizenship documentation if not born in Australia.
